That was a strategic move.

That 4th down play was meant to see if they could draw the defense offsides. If not, there's no point in wasting a timeout, so take the five yard penalty and punt instead.

And Koch nailed a 58-yarder anyway, so makes no difference other than putting the ball in Cleveland's hands.

Yea the hard count doesn't really work if Flacco has to run to the sideline to hear the play. At least Koch made up for that 5 yard penalty.
