Ravens License Plates

Quick question. The form for the Ravens licenses plate; what part do I need to fill out? There seems to be 3 sections to it. It looks like the DMV fills out the bottom part, I fill out the middle, but what about the top? Do I put in the Ravens info there, or do I send it in with that part blank?

Upon further review I think we only filled in the top part. Its really nice to see more and more people getting the tags. I love them and can remember my tag number now
